CONTRAINDICATIONS: WHEN NOT TO RECEIVE MLD

Don’t Do It: Absolute Contraindications!
These conditions are absolute stops. MLD should not be performed if any of the following are present:
- Acute infection or inflammation: active fever, contagious illness, cellulitis, or any active bacterial or viral infection. Increased lymphatic flow can spread pathogens or toxins through the system more rapidly.
- Active thrombosis: deep vein thrombosis (DVT) or any known blood clot. Increased circulation can dislodge a clot, creating a life-threatening situation.
- Acute cardiac conditions: cardiac decompensation, acute cardiac edema, or congestive heart failure. MLD increases the fluid load returning to the heart. In an uncompensated cardiac state, that additional load is dangerous.
- Severe kidney failure: significant renal dysfunction means the body cannot process the increased fluid movement that MLD produces.
- Undiagnosed lumps or active malignancy without oncology clearance: untreated cancer is an absolute contraindication. *However, MLD can be appropriate and beneficial for oncology clients undergoing chemotherapy or post-treatment, with proper medical clearance.
Proceed with Caution: Relative Contraindications
These conditions require medical clearance before receiving MLD. They aren’t automatic disqualifiers, but they require a conversation with your doctor first:
- Thyroid conditions: hyperthyroidism in particular, especially when work involves the anterior neck.
- Pregnancy: Pregnancy is not a contraindication… but it’s not simple either. MLD is generally safe and often genuinely beneficial during healthy pregnancies. It can reduce swelling, support nervous system regulation, and improve circulation in ways that feel meaningful, especially in the third trimester. Clients who already have MLD in their regular routine can often continue through pregnancy with physician awareness and no interruption.
- The caution comes with context. High-risk pregnancies involving preeclampsia, hypertension, placental concerns, or medically managed edema require explicit physician clearance before any lymphatic work. Increased fluid circulation is the mechanism that makes MLD effective… and in those specific situations, it’s also the reason physician guidance matters. Any pregnancy, routine or otherwise, should be disclosed at intake. From there, we figure out together what’s appropriate.
- Chronic conditions including diabetes, asthma, or significantly high or low blood pressure.
- Recent surgery, healing incisions, implants, or fat transfers. Post-operative MLD with medical clearance is not just acceptable, it’s often highly beneficial. Starting daily MLD treatments as soon as cleared can meaningfully reduce bruising, swelling, and inflammation, and support faster recovery.
