This form is required for all clients who have been in an accident, suffered a major injury, or have had a recent surgery. It must be completed before your first appointment.

A written physician clearance is required before services will be provided. Incomplete forms cannot be accepted.

COMPLETE POST OP + POST ACCIDENT CONSENT

This form is required for all clients who are currently undergoing or have recently completed cancer treatment, including chemotherapy, radiation, immunotherapy, or surgery. It must be completed before your first appointment.
